All new patients are required to complete the online intake form prior to their visit to ensure accurate screening and proper scheduling. All information submitted is kept strictly confidential.

A credit card is required at the time of scheduling.

If you are unable to make your appointment, we ask you to let us know 24 hours in advance to reschedule or cancel your appointment. No Show or missed appointments will result in your account being charged the full cash price, for all appointment types including new patient intake appointments, and will not be covered by your insurance.

Please also keep in mind that our office can not guarantee insurance coverage but we can verify your insurance for you. Our office accepts and processes PPO health insurance policies as long as they have out-of-network benefits. Unfortunately, we are out-of-network with almost all insurance companies. Feel free to email your insurance card and insurance information (full name of subscriber and DOB) to the front desk staff at admin@henstockchiropractic.com if you would like us to verify your coverage and before your appointment.
